They believe Fritzl, 73, may have kept his granddaughter Kerstin, 19, as a sex slave in the windowless dungeon he constructed under his house in the town of Amstetten, west of Vienna.
Medics and detectives have been unable to speak to Kerstin who is in a coma and has suffered multiple organ failure after spending her whole life underground.

Albert Reiter, the doctor treating her, said: "She is in a grave condition and it's impossible to interview her because she is unconscious. We have to wait for a later time to make such investigations."
Fritzl imprisoned his daughter Elisabeth at the age of 18 in 1984 and used her as a sex slave, fathering seven children by her.
Before that he had fathered another seven children above ground with his wife Rosemarie, 68.
Chief investigator Franz Polzer said: "His motive was to recreate once again the situation he had with his first family, the legal family, but this time with a good looking young daughter."
By the time Elisabeth, now 42, was rescued from her prison her looks were ravaged. She was grey, aged beyond her years and looked like a sister to her mother Rosemarie.
Detectives now fear that, as Elisabeth aged, Kerstin may have become Fritzl's next victim.
Elisabeth and two of her other children Stefan, 18, and Felix, five, who were kept underground their whole lives, are said to be improving at a clinic outside Amstetten. Staff have given them a fish tank and Felix has been given a teddy bear.
Stefan and Felix have met their three siblings - Lisa, 16, Monika, 15, and Alexander, 12, - who were taken out of the cellar as infants.
The seventh child died young and his body was thrown into an incinerator by Fritzl.

His lawyer Rudolf Mayer, who visited him in prison yesterday, told the Daily Telegraph: "He's ashamed. He regrets what he has done. He wants a psychiatric expert to look into his soul. He is ill."
Dr Mayer said his client would plead insanity. Fritzl is refusing to come out of the cell he shares with another prisoner, who is accused of drug offences, after other inmates threatened to kill him.
